Fellowship Application

Concordia provides support for medical fellowship programs to assist in the cost for this additional education. Fellowship support may be provided only in pulmonology, thoracic surgery, and gastroenterology to individuals who have successfully completed their residency training at a US-accredited institution. An academic or training institution must select the individuals receiving fellowship support.

Concordia cannot be the sole supporter of any fellowship program. The fellowship program curriculum must be entirely designed by the academic or training institution. Payment will be given to the sponsoring institution and not to the individual.

Application Process

Applications are evaluated on a rolling basis. Every attempt will be made to provide a response to your request within 60 days of submission. All requesters must sign and return the letter of agreement before any funds are issued.

Concordia, at its sole discretion, may reject an application for various reasons. Funding restrictions include:

  • Overhead or indirect costs of any kind
  • A position that is funded by payments received from an insurance company including government healthcare programs (e.g., Medicare and Medicaid)

For fellowship applications, please upload the following information:

  • W-9/W-8BEN form
  • Detailed itemized budget for the entire program
  • Statement that the funding will not be used for any services that are billed to an insurance company, including federal healthcare programs (e.g., Medicare and Medicaid)
