Charitable Donation Application

Concordia supports disease state education, patient education and outreach, and scientific and medical meetings, symposia, and conferences that align with the therapeutic areas that Concordia is involved in. Concordia occasionally funds requests for general organizational support for an unspecified purpose; however, these organizations must be non-profit and must not purchase products from Concordia. To receive a charitable donation from Concordia, your organization must be a tax-exempt entity, such as a patient advocacy organization (501(c)(3)), a cultural or civic organization (501(c)(4)), or a professional medical association or civic organization (501(c)(6)).

Application Process

Applications are evaluated on a rolling basis. Every attempt will be made to provide a response to your request within 60 days of submission. All requesters must sign and return the letter of agreement before any funds are issued.

In order for your application to be considered, funding must not be used

  • for religious purposes
  • for individuals or memberships
  • for advertising/campaigning of any sort
  • for continued medical education
  • for any illegal purpose

Please upload these required documents:

  • W-9/W-8BEN form
  • 501(c)(3), 501(c)(4), 501(c)(6) designation from the IRS
  • Meeting agenda (if applicable)
  • Detailed itemized budget (if applicable)
  • Promotional materials for event
